To Apply for Child Support Services
You may apply for Child Support Enforcement services if:
- You are the parent or caregiver of a child who needs or is owed child support.
- You are a parent who is owed unpaid support.
You do not need to apply if:
- You have recently applied for assistance from the state.
- You now receive public assistance from the state.
We will automatically provide child support services.
You may not need to apply if:
- You received public assistance or child support services in the past. Your case may still be open.
- If you received child support services from another state, you may need to contact that state to find out if your case is still open.
Follow these steps to Apply for CSE Services:
NOTE: Please read all of the instructions before completing the application for services. The forms for you to download and complete are at the bottom of this web page.
- Fill in the application online, print it from your computer and sign it by hand. You can also print the blank application and fill it in by hand.
- Complete the application to the best of your ability. We need as much information as you have about yourself, the other parent(s) and the children.
- The application only allows you to provide information on one child and one parent at a time.
You will need to complete a separate Parent Information or Child Information form if:
- There is more than one child who needs services.
- There is more than one possible father.
- There is both a legal father and a biological father and you have only named one.
- You are a nonparent caregiver and want us to collect support from both parents.
- Complete a separate Parent Information form for each additional parent (or possible parent) who does not live with you.
- Complete a separate Child Information form for each additional child who needs services.
Example: If you have two children and there are two possible fathers, you must complete:
- All pages of the Application for Services
- One additional Parent Information form
- One additional Child Information form
- When you finish, with your application and additional forms, check to make sure you provided the following information:
- The type of service you are requesting. Select either full service or location only.
- Full names for you, the other parent(s) and the children.
- Dates of birth for you, the other parent(s) and the children.
- Social Security Numbers for you, the other parent(s) and the children. We keep this information safe. We use it only to identify individuals and to locate and collect child support from the other parent.
- Addresses for you and the other parent(s).
- Sex and race for you, the other parent(s) and the children.
- Employer(s) for you and the other parent(s).
- Any other names used by you or the other parent(s).
- Phone numbers for you and the other parent(s).
- Each child’s birth certificate if the child was not born in Florida.
- Any paternity judgments, support orders, payment records or written agreements between you and the other parent. If you don’t have copies, we need to know the county, the state and the approximate date of the order.
- A copy of the health insurance card if the child(ren) is insured.
Please be sure to send us copies of the documents we need along with the application. You may need to contact other people or the clerk of court to get the information.
- Sign and date the application. If you forget to sign, we will return the application and the forms. Until we get a signed application we will be unable to help you.
- Return the signed, completed application, any additional Parent Information or Child Information forms and other documents to the nearest child support office in Florida.
- If you have a Florida order, send your application to the child support office for the Florida County of the order.
- If you do not have a Florida order, send your application to the child support office for the Florida County where the other parent is living.
- If you do not know where the other parent is living, send your application to the state program office at: Department of Revenue, Child Support Program, P.O. Box 8030, Tallahassee, Fl 32314-8030.

- We will start when we get the signed application and all the information we need. This should take no longer than 21 calendar days.
- Under the Application Links section below, click Application for Child Support Services form to get started. This will open a blank application on your computer. You will need Acrobat Reader 5.0 to complete the application online.
Note: You will not be able to save a copy of the completed form to your computer after you fill it in. If you want a copy, you must print an extra or photocopy the one you complete.

After You Sign Up for Our Services
We may ask you for more information or forms after you send us your completed application. It is important that you give us the information we need right away. We may ask you for:
- Your current income information. This could include check stubs; W-2 forms; 1099 forms if self employed and federal income tax returns for the past 3 years.
- A completed Financial Affidavit
- A completed Paternity Declaration
Tell us right away about any changes in:
- Your address or employment
- The other parent's address or employment
