Register to Collect and/or Report Tax
Information Needed
Before you begin, you should gather specific information about your business activities, location, and beginning date. You will need the same information required to complete the paper application (Form DR-1):
- Full name, social security number, home address and telephone number, and driver license number of the business owner, or a corporate officer
- Federal Employer Identification Number (FEIN), if applicable, for the business entity; if you are registering for Unemployment Tax, you must have an FEIN. If you do not have an FEIN, visit the IRS web site to apply for and receive your number online
- Physical and mailing address of the business entity
- If incorporated, date of incorporation, fiscal year ending date, and Florida corporate document/registration number (assigned by Secretary of State, Division of Corporations)
- If you rent your business location, and do not operate from your home, the name, address and telephone number of the property owner or manager
- If you are registering for unemployment tax (UT) and you lease your employees from an employee leasing company, the name, FEIN, UT Account Number and DBPR license number of the leasing company
