Download Current Forms and Documents

The Florida Department of Revenue provides its most common requested tax forms for download. These files are in the pdf file format and can be viewed using the free software Adobe Acrobat Reader version 4.0 or greater. Any forms submitted to the DOR must be printed on a laser or inkjet printer with a resolution of 300 dots per inch or higher. Forms printed on a dot matrix printer CANNOT BE ACCEPTED.

You may also order forms directly from the Distribution Center.
